Registration Options
There are two registration options:
- Register online and pay with a credit card.
- Register by mail and include a check or purchase order.

- To pay by check or purchase order, print and complete the printable registration form and mail it with your check (payable to Children and Family Futures) to Children and Family Futures, P.O. Box 714356, Columbus OH 43271-4356.
- For purchase order payments, the CFF tax ID number is 33-0731001. Please email skapsner@cffutures.org if you need a W-9 form.
- Please allow seven business days for receipt of your mailed registration.

- Mail-in registrations postmarked after January 19 must pay the on-site registration fee.
Please Read Before Registering:
- Use correct upper or lower case letters in your name, address, and other information. Information entered here will be used on your name badge and other documents.
- Cancellation and Refunds: All changes and cancellations must be made in writing and submitted via fax to 330-963-0319 or via email to cff@experient-inc.com. Cancellations must be received by December 1, 2006 and are subject to a $100.00 administration fee. If you cannot attend the conference, you may send a substitute. Please submit the request to the fax number or email given above. If you cancel after December 1, 2006 or do not attend, you are responsible for the entire fee.
- Registration for the conference does not include a hotel room. See the Hotel and Travel page for information on reservations.
- Continuing Education Units (CEUs) are available for a $30 fee. You can add this cost to your registration fee, or you can pay for CEUs at the conference. CMEs and CLEs are also offered. See the Continuing Education page for more information.
